Residential Rehabilitation (Behavior Focus Group Home)

At our group home (St. Clair Group Home), we offer around the clock care services to ensure that our clients get the attention they need to reach their goals.

Our group home is wheelchair accessible.

We provide medication administration,  supportive listening, feeding residents, laundry, assisting residents with activities of daily living – e.g. assisting with bathing, grooming, dressing, facilitating community and recreational outings.

Respite

Graceful Homes provides respite care services. Respite services (Personal supports) involves our team member providing temporary relief for a primary caregiver, enabling them to take a much-needed break from the demands of care giving for a sick, aging, or disabled family member
